of the causes of unnatural death
a first instance cause would be a mortality
occurring be when someone was
installing some high voltage
electrical equipment
and had to test it
a second instance would be the
equipment used to test it
has a fault with it
a third instance would be the assembly
of the test equipment was the
cause of the problem
it appears that the causes don't go
beyond four or five instances in
the chain of cause and effect
